CORI / Kids Safe

To All Adult Volunteers for the Fall 2007 Season:

IF you have not yet filled out a CORI/KIDSAFE form you are required to do so before the start of practices and/or games of the Fall 2007 season. If you previously filled out the CORI/KIDSAFE form in prior seasons, you do not have to submit another form.

The Massachusetts Youth Soccer Association is requiring a CORI/KIDSAFE form for each adult volunteer, coach, administrator, or anyone who will be involved with youth soccer.

This form must be filled out and sent in to the MYSA before you are allowed to take part in the season. MYSA offers three (3) choices for completing and filing this form. Please access their web site: www.mayouthsoccer.org and click on the heading “Resources”, scroll down and click on “Registration”. The first section deals with the CORI/KIDSAFE form.

• You may fill out the form online at the MYSA website and e-mail it directly to MYSA
• You may access and complete the Word version at the HYS website and e-mail it to MYSA
• You may access and complete the .pdf (Printable Form Document) format and mail it to MYSA, 2444 Old Union Turnpike, Lancaster, MA 01523

We recommend that you complete this form online at the MYSA website where you can immediately e-mail the completed for to MYSA. If you experience any difficulty accessing the online form through the MYSA website, you will also find the Word and .pdf versions on our site. When filling in any of the forms, please include the club name as: Hingham Youth Soccer, Inc.

This form is not an option. You will not be allowed to coach or participate as a volunteer until this information is recorded with the MYSA. Your immediate response is appreciated.
